Community care and support options

There are a range of community service providers that can assist people living with dementia to remain relatively independent in their own homes. The GP has a pivotal role in community care and carer support and should encourage patients and family to access community services and assist with referrals where required.

Community care and carer support information can be found on the next level of the pathway. The Commonwealth National Respite Carelink Information Line can also link patients and family to community service providers.
