Aged Care Assessment Service (ACAS)
An Aged Care Assessment should be considered post-diagnosis for people with complex care needs. The role of the Aged Care Assessment Service (ACAS) is to comprehensively assess the needs of frail, older people and assist them to gain access to the most appropriate types of care when they are no longer able to manage at home without assistance.
An ACAS assessment of a person's care needs includes:
- Medical issues and concerns
- Activities of daily living including personal care and domestic activities
- Formal and informal support networks
- Carer needs and concerns
- Client wishes, strengths and needs
An ACAS assessment is mandatory for entry to Commonwealth funded residential care facilities, the Transition Care program and Commonwealth Home Care Packages.
Triggers for referral
- Patient’s care needs are significant
- An assessment is required for restorative care options or a Commonwealth-funded service such as the Home Care Packages Program
- Carers are encouraged to be at the assessment, and referrals can be made to support the carer as required[1]
