This number is a central point of contact for assessment to determine eligibility for a home care package.
Coordinated packages of care exist for people with dementia who have complex care needs and who wish to remain living at home. A Home Care Package provides services that can:
- Help the person to stay at home for as long as possible
- Give people choice and flexibility in the way that care and support is provided
There are four levels of Commonwealth funded Home Care Packages. The four levels are designed to give people care that supports current need but can also be changed if needs increase later.
Home care Levels 1 and 2 help people with basic or low level care needs. Level 3 and 4 help people with intermediate to high care needs. Additional government funding is also available to support people living at home with dementia through a new Dementia and Cognition supplement.
There are also Consumer Directed Care Packages (CDC) which are designed to give people with dementia and their carers greater control over the design and delivery of the care and services that they receive.
To access a Home Care Package, patients must be assessed as eligible by an Aged Care Assessment Services (ACAS). Patients must also prefer to remain within their community, prefer to receive a Home Care Package, and be able to live at home with the assistance of a package.
