District nurses provide a range of services to support the person with dementia in their own home, such as continence assessment, medication management and personal care for people with unstable health status or complex needs.

- Nutrition and hydration
- Helping manage medicines and medication education
- Health information and education
- Assistance with continence issues
- Enabling patients to live safely in their own home
- Complete a comprehensive and holistic assessment and referral to a range of community services.
- Personal care requiring nursing involvement due to unstable health and / or complex needs
