HACC Assessment
Referral for a Home and Community Care (HACC) Assessment should be considered post-diagnosis. If eligible, the assessment process seeks to assist both the person with dementia and their carer to get the right supports to help deal with everyday issues and get timely information and assistance to navigate the service system.
An initial HACC Assessment may be undertaken which is an holistic assessment that incorporates care planning, service-specific assessment for the person and their carer, and care coordination as required
Possible support
- A review of both the person with dementia and their carer’s needs can help the person with dementia to be as independent as possible in the community
- Referral to an occupational therapist for a home-based assessment to identify ideas and possible equipment
- Plan care arrangements on the basis of improving people’s functioning, building on interests, strengths and lifestyle patterns.
- Suggestions for other transport options to deal with the person with dementia’s concerns about loss of independence if unable to continue driving.[1]
Triggers for referral
- Patient is unable to cope independently with everyday activities such as housework, showering and dressing
- Patient has poor nutrition
- Patient is becoming socially isolated, due to their inability to drive or use public transport
- Carer stress is evident, where the patient has a carer[2]
