Planned activity group

For people with dementia and their carers, maintaining social contact and engaging in meaningful, enjoyable activities in the local community is vital to wellbeing.

Encourage the patient and carer to access activities and outings provided by HACC funded planned activity groups.

Triggers for referral

The person is socially isolated due to disability, memory loss, or an inability to drive or use public transport. Their carer may need support and respite from their caring responsibilities.[1]
